Rapid Response Community Nurse

Are you an experienced Registered General Nurse or a newly qualified nurse seeking an exciting opportunity to work within a Rapid Response Nursing Team across Derby City?

Main duties of the job

The Service is to provide acute (short term) Nursing to patients in their own homes and Residential Care Home settings, to support self care,  prevent hospital admissions and facilitate early discharge.

These empowering nursing roles within the Rapid Response team will involve you, using your clinical and leadership skills in response to:

  • Urgent and next day community nursing referrals
  • Holistic Patient Assessment and effective caseload management
  • Co-ordinating the Nursing rapid response during the shift
  • Developing your skills and knowledge in effective triage

The service is based in the community 8am till 6.30pm,  7 days a week.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

  • RGN
  • Evidence of recent relevant training
  • Evidence of community experience placement/job role
Desirable criteria
  • Experience in rapid response service.
