Job overview

We have an exciting opportunity for a highly motivated, enthusiastic Nurse to join the DCHS Community Continence Service.

You will be able to demonstrate a genuine interest in the field of continence and an ability to provide a high standard of continence care within a variety of settings. 

Our team focus is on bladder and bowel health promotion and therefore it is essential that you have a keen interest and want to make a difference and ensure clinical excellence is achieved.

The successful candidate will also have experience of Community working and will be able to train and educate clients, carers, other health care professionals and the general public in continence promotion and the management of incontinence.

You will need to possess excellent communication skills and the ability to deal with challenging circumstances at times.

Main duties of the job

To provide a specialist continence service across a defined geographical area in a variety of settings in order to promote a high level of service and awareness to clients, carers and other health care professionals.

 This is an ideal opportunity to become part of a specialist team which makes a real difference to people’s quality of life.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

To provide a comprehensive pro-active continence promotion service within a variety of community settings with initial responsibility for the continence assessment and evaluation of care.

 To support clients and carers with treatment options to promote continence and manage incontinence effectively.

 To identify need, order equipment and  provide advice, support and demonstration to the patient/carer on the use of the equipment to ensure a personal duty of care and compliance with health and safety policies.
